The first step to getting started in our program is to attend an upcoming free educational class with Dr. Davis. At this class, you will learn about all available surgical options, how surgery works, why it works, risks, benefits, what to expect regarding weight loss and how to move forward quickly. You will receive our “getting started” packet, making it as smooth and easy as possible to get you approved for surgery with your insurance company and then to schedule your surgery date. Your attendance of this class is mandatory, as your education is of utmost importance to your success. This is the first step to making the change to a healthier you! When you book your appointment to see Dr. Davis, please print off our new patient forms, complete them, and bring them with you to your first consultation.

We understand that we are asking a lot of information of you, but this will help insure your benefits are properly verified, your medical and health history is documented appropriately, and we are able to get your chart started, thus expediting your visit with us and moving you closer to surgery and to a new, healthier you!
